Patient Registration & Release of Medical Records

Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit and please bring your driver’s license and most recent insurance cards. Completing the forms prior to your visit will help speed up the new patient registration process.
If you would like to request a copy of your medical records, please completely fill out the forms below and bring it to the Main Clinic. Our Medical Records department will be happy to assist you. (You will be required to provide proof of identity at the time of submission.)
