Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the patient registration form. Please be sure to bring it with you to your first visit. Completing the form prior to your visit will help speed up the new patient registration process. You will also be able to request access to our patient portal web site at that time.

Existing patients

If you are already an established Silver Health CARE patient, you may request an appointment, request a refill, ask a medical question, request a medical record, ask an insurance question or ask a general question through our new PRIME patient portal at myhealthrecord.com.

If you do not have access now, on your next visit to Silver Health CARE, ask the front desk about signing up for our new patient portal.

Urgent CARE

Silver Health CARE is available when you need us, with extended walk-in hours 7 days a week in Silver City at our Urgent CARE Clinic. Come to our Urgent CARE Clinics instead of the emergency room for illnesses or minor injuries. You will see a provider in minutes, not hours, and save time with diagnostic services available right here in our facility. Our services are competitively priced to save you money over a trip to the ER.

Medical Emergencies

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Silver Health CARE, and they will contact us.

Outside of Our Regular Business Hours

When you need us outside of our regular business hours, call (575) 538-2981 and our answering service will gladly assist you.
