Registration: Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the patient registration form. Please be sure to bring it with you to your first visit. Completing the form prior to your visit will help speed up the new patient registration process.
Records Release: If you would like a copy of your medical records, please fill out the Release of Information form completely and bring it to the Main Clinic. Our Medical Records department will be happy to assist you.
At Silver Health CARE, we accept workers’ compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at (575) 538-2981 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.
The billing office for Silver Health CARE is located at 608 Winifred Street in Bayard. Bills can be paid in person at this office or at any one of our clinics. For more information on our policies, please see Silver Health CARE’s Financial Policy page.
If one of our primary care providers suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require three business days for the processing of routine referrals.
Physicians at Silver Health CARE are affiliated with Gila Regional Medical Center.
If you require a prescription refill, contact your pharmacy and have them request a refill. All local refill requests need to come from the pharmacy, not from the patient. Please allow two to three business days for the request to be completed.
Please note: if you have not seen your doctor in over a year, you will need to make an appointment before your prescription can be refilled.
